What is Documented?
Documented is a privacy-first iOS app that turns your daily experience of chronic illness into structured evidence for disability benefits claims. It maps your entries to the assessment criteria used by PIP (UK) and SSDI (US), so when it’s time to file or appeal, you have a clean, contemporaneous record.
Which benefits schemes does it support?
The first release focuses on Personal Independence Payment (UK) and Social Security Disability Insurance (US). Other UK schemes (ESA, Universal Credit LCWRA) and other countries may be added based on user demand.
Is this legal or medical advice?
No. Documented helps you keep an evidence record. It is not legal advice and not a substitute for a benefits advisor, welfare rights officer, or solicitor. For complex claims, get qualified support.
Will my evidence be accepted by an assessor?
Contemporaneous symptom and limitation diaries are widely recognised by courts, tribunals, and assessors as objective evidence. The strength of any individual claim depends on consistency, comprehensiveness, and the underlying medical case — Documented helps with the first two.
Is my data private?
Yes. Documented has no accounts, no analytics, and no Cocoacorn servers. Data stays on your device unless you choose iCloud sync in your personal Apple account.
